These forms are required to be used for all communication to the ES&CTTA that the forms cater for.

 Contact Form For those interested in playing pennant table tennis.  
 Feedback Form To provide some feedback about the ES&CTTA Website, RatingsCentral, or any other matters.  
Eley Park Contact Form For those interested in playing pennant table tennis at Eley Park.  

Players
Membership Form For players who, prior to the season beginning:

  • are new players to the ES&CTTA ; or
  • are former ES&CTTA players that have not played in this or the last calendar year.
 Prior to the season beginning
New Player Permit For fill-ins during the season who:

  • are new players to the ES&CTTA; or
  • are former ES&CTTA players that have not played in this or the last calendar year.
 Due by 12.00 noon on Saturday following the match.
Match Permit For fill-ins during the season who:

  • are previous players in the ES&CTTA; and
  • have played in this or the last calendar year.
 Due by 12.00 noon on Saturday following the match.
Match Permit Form for players based at another venue For fill-ins based at another venue Approval required prior to playing.
Player Transfer to another Team Form For players wanting to move to a team within the grade or to a lower grade during the season Approval required prior to playing.
Player Transfer to another Venue Form For players wanting to move to be based at another venue during the season Approval required prior to playing.
Fill-In Exemption Request For fill-ins who break the 6 player limit or who are filling in for another team in the same grade. Due by 12.00 noon on Saturday following the match.

Teams
Team Roll Over Form To keep a pennant team  Due by the Grand Final of the current season.
Team Entry Form To enter a pennant team  Due by the Grand Final of the current season.
Multiple Team Entry Form For venue managers to enter all their teams at once Due by the Grand Final of the current season.

Other Notifications
Match Postponement Notification Form For those exceptional times when everyone on both teams are unavailable and the match can be played at a later date. Due by 12.00 noon on Saturday following the scheduled team match.
Match Disputes and Protests Form For disputing the score entries and protesting all sorts of things. Due by 5.00pm on Monday following the match.
Late Result Fine Appeal Form For winning captains who didn’t get the scores in by Saturday 12 noon due to exceptional circumstances that warrant consideration. Due by 5.00pm on Monday following the match.
Walkover Fine Appeal Form For captains who didn’t get there team together for a match, but believe that there are exceptional circumstances that warrant consideration. Due by 12.00 noon on Saturday following the match.
